The subject line should look like
Type of Media: Name of Item
e.g.: Film: Hugo (2011); Book: The Lord of the Rings (JRR Tolkien); Music: Help! (The Beatles); etc.
Films are denoted by year just due to the tendency of films to repeat their names a lot!

Above the cut should look like:

Contains: Trigger with brief description (such as: graphic/non-graphic, discussion of, off-screen/on-screen.)
e.g.: Contains: Graphic misogynistic language; Contains: Stereotypical villainous trans*person; Contains: Disability as humor.
The intent of the "contains" section is to be useful without being spoiler-y (so that people can be unspoiled if it is not triggery to them). None of these actually tell you much about the plot/content of the story!

Below the cut should look like:

Context: Where/how the potentially triggery material takes place! Please make this as non-graphic as possible, while still explaining the extent to which the triggery material occurs. The non-graphic is because the people clicking this cut are probably those who are affected by this trigger. If you are working from a live-action show/anime/manga/book series/etc. it would be great if you'd mention where this occurs, and if the episode/arc/whatever it occurs in can be skipped.
An example for a movie is the one I wrote about Hugo (2011) (tw: disability as humor). Don't worry if I have to send a version back to you! I am running my own posts past people, generally, to make sure they're okay.

The current list of tags are the preferred terminology within the comm. Content warnings should include jokes based on those precepts, bullying, and more direct references/usage of them. Described below are the two which seem to be the most likely to be confusing. Any questions/concerns/additions, feel free to comment on this post.

Remember: nothing is included as a moral judgment; rather these are the warnings I have most commonly encounter being requested in fandom. If you enjoy reading/viewing things that include these topics that's okay, this is just for people who don't. (BDSM, for example, is by definition consensual, it's only included because the visuals do trigger some people. Triggers are not logical.)

Gender Topics
Anything that doesn't fit within misogyny or trans*phobia -- for example, the rigid enforcement of gender stereotypes, erasure of non-binary people (genderqueer, agendered, etc.), or things that are more amorphous.

Sex Topics
For topics that fall outside or around the topic of assault -- power imbalance (both implicit as in teacher/student, and explicit as in consensual dominance/submission), other BDSM elements, underage. General sexual imagery can also fit here -- though easier to find warnings about than other things, it can be a trigger.
